Waiver
Please read carefully and fill out the waiver below.
By participating in any capacity in the sports organization's athletic sports program, related events, and activities, I acknowledge, value, and consent to the following:
I am fully aware and willingly accept all the risks associated with my involvement, whether they are known or unknown, even if they result from the carelessness of the individuals or entities being released from liability, or any other party. I also take complete responsibility for my participation.
Engaging in this program exposes me to substantial risks of injury and illness, including but not limited to communicable diseases like MRSA, influenza, and COVID-19. These risks encompass the potential for permanent paralysis and death. Although specific regulations, equipment, and personal discipline may decrease these risks, there remains a possibility of serious harm and illness.
I am willingly consenting to abide by the established and customary terms and conditions for participation.
If, at any point, I notice any exceptionally significant hazards while present or participating, I will immediately withdraw myself from the activity and promptly inform the nearest authorized individual.
On my own behalf, as well as on behalf of my heirs, assigns, personal representatives, and next of kin, I hereby release the sports organization, its officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of the premises where the event takes place (collectively referred to as "RELEASEES") from any and all liability, injury, illness, disability, death, loss, or damage to myself or my property, regardless of whether such harm arises from the negligence of the RELEASEES or any other cause, to the maximum extent permitted by law.
I have carefully read and fully understand the terms of this agreement, including the release of liability and assumption of risk. I comprehend that by signing this agreement, I am relinquishing substantial rights willingly and voluntarily, without any form of coercion or inducement.
FOR PARTICIPANTS UNDER THE AGE OF 18 (MINORITY AGE) AT THE TIME OF REGISTRATION
I, as the parent or legal guardian with full responsibility for the aforementioned participant, hereby confirm that I have thoroughly reviewed and explained the contents of this waiver/release to my child/ward. This includes discussing the potential risks associated with the activity, as well as emphasizing their obligation to abide by the established rules and regulations. Furthermore, my child/ward comprehends and willingly accepts both the risks involved and their corresponding responsibilities. As for myself, my spouse, and my child/ward, we provide our consent and agreement to the aforementioned release on behalf of all the Releasees. Additionally, we release and undertake to indemnify and protect the Releasees from any and all liabilities that may arise as a result of my minor child's/ward's participation or involvement in these activities, even if such liabilities stem from the negligence of the Releasees, in accordance with the maximum extent allowed by law.
